This promotional raffle is for all Doctors and their staff members. When a doctor or a staff member refers a patient to our office that person's name will be entered to win a 2 guest 2 night stay at The Hyatt in Estero. You will also receive a $100 credit to enjoy the spa activity of your choice during you stay. Send the patient you refer to our office for a free initial consultation with a ticket, and your name will be entered to win! The more you refer the more chances to win. There is no limit to the number of times your name can be entered. The Drawing will be held on July 1, 2009.
3) Patient Referral Contest!
We our holding a raffle for all of our patients. To enter all you need to do is refer a patient for a Free initial consultation. When they start treatment, your name and the name of your referral, will be entered to win a tickets for 4 to Disney World! Pick up your tickets to enter at your next appointment.
