This policy explains how Kriger Orthodontics (Maryann Kriger, DDS, PA) collects and uses information through www.bonitaortho.com, including its forms, chat, cookies, and similar technologies.
Your cookie choices: optional Analytics and Marketing are off until you allow them. Use Cookie settings in the footer of any page to review or change your choice.
1. Website privacy and patient information
This policy covers the public website. Our separate HIPAA Notice of Privacy Practices describes the use and disclosure of protected health information and your rights concerning patient records. Cookie preferences are separate from consent to treatment, communication preferences, and any authorization required for the disclosure of protected health information.
Please avoid sending detailed medical histories, insurance identifiers, or other sensitive health information through general website forms, chat, or ordinary email. Contact our office for guidance on sharing patient information.
2. Information collected through the website
- Information you provide: when you request a consultation or contact us, you may provide your name, phone number, email address, preferred office, treatment interests, and the contents of your message. If you use a feature that accepts uploads, the files you submit are also provided to the practice and the service used to receive them.
- Technical information: connections to this website and its embedded services transmit information such as your IP address, browser and device details, requested pages, and referral information. Hosting, security, and delivery services may process this information to operate the website.
- Optional tracking information: if you allow Analytics or Marketing, the corresponding tools may collect cookie or device identifiers, pages viewed, interactions, and campaign information.
- Cookie preferences: the website stores which optional categories you have allowed or rejected so it can apply your choice on later visits.
3. How information is used and shared
Information submitted through the website is used to respond to inquiries, coordinate consultation requests, and communicate about the services you request. Technical information supports website delivery, maintenance, security, and spam prevention. Optional analytics helps us understand website use; optional marketing tools support advertising, campaign measurement, and call attribution.
Information may be processed by the providers that supply our website hosting, forms, chat, communication, security, analytics, and advertising services. When a third-party resource loads, your browser connects to that provider and may send technical information directly to it. Advertising providers may associate identifiers and browsing activity with activity on other websites or devices, as described in their notices. Information may also be disclosed when required by law.
The website currently includes these integrations:
- Forms and chat: LeadConnector / HighLevel supports embedded consultation forms and chat.
- Security and website delivery: Google reCAPTCHA helps protect forms from spam; externally hosted fonts and assets support the appearance and operation of the website.
- Optional analytics: a Google Analytics integration.
- Optional marketing: Google Tag Manager and advertising tags, Meta Pixel (including the Pixel Cat integration), AdRoll / NextRoll, Vibe, APXL, PPCmate, and Marchex call-attribution scripts.
Provider information is available in the Google Privacy Policy, Meta Privacy Policy, NextRoll Service Privacy Notice, and HighLevel Privacy Policy. Contact our practice about information you submit through our forms or chat; provider notices do not replace this policy or our patient privacy notice.
4. Cookies and your settings
Cookies are small pieces of information stored by your browser. Related technologies, including pixels and scripts, can transmit information about a visit without storing a cookie. Our cookie notice offers three categories:
- Necessary — always active: storage and features needed for website operation, security, and remembering your cookie choice.
- Analytics — optional: tools used to understand how visitors use the website.
- Marketing — optional: tools used for advertising, campaign measurement, and call attribution. The website’s Google Tag Manager integration is included in this category.
Accept all allows both optional categories. Reject all keeps both off. Manage options lets you select categories and choose Save my preferences. Closing the notice without making a choice does not grant consent.
Your selection is saved in the first-party cookie bonita_cookie_consent for up to 180 days after you save it, unless your browser deletes it sooner. It contains the preference format version and your Analytics and Marketing choices; it does not contain your name, contact details, or patient information. Deleting this cookie causes the notice to appear again.
You can change your selection at any time using Cookie settings. When you turn off a previously allowed category, the page reloads to stop the associated scripts. The website also attempts to clear recognized first-party tracking cookies. This cannot remove cookies held on another provider’s domain or undo information already sent. You can delete cookies and other site data through your browser; doing so may also remove preferences or affect website features.
Embedded services remain available: the cookie controls manage the Analytics and Marketing scripts listed above. They do not disable appointment forms, chat, externally hosted fonts, or spam protection. These services may receive technical information when they load and may use their own cookies or storage. Google provides further details about its use of cookies and reCAPTCHA.
5. Retention and security
The 180-day period above applies to the website’s consent cookie, not to every website record or third-party cookie. Retention of inquiries, submitted files, technical records, and provider-held information depends on the service, the purpose of the information, and applicable recordkeeping requirements. Contact our office for questions about a particular submission. Patient-record retention is separate from cookie preferences.
This website uses HTTPS for connections to its pages. No website, email service, or method of electronic transmission can guarantee absolute security. Cookie settings do not make general website forms or ordinary email a secure channel for sharing sensitive health information.
6. Your choices and privacy requests
You can browse without allowing optional Analytics or Marketing, change cookie preferences at any time, and choose whether to submit an inquiry. Browser controls can also limit cookies and other storage, although those controls may affect forms and other features.
Contact us to ask about information you have submitted or to request access, correction, or deletion where applicable. Available rights and any exceptions depend on applicable law and recordkeeping obligations; we may need to verify your identity before responding. For requests involving patient records, please refer to our HIPAA Notice of Privacy Practices.
Cookie preferences do not change messages or appointment communications you separately requested. To change those preferences, follow any instructions in the message or contact our office.
7. Children and information about others
A parent or guardian should handle website inquiries about a child’s care. Please avoid including a child’s detailed health information in general forms or chat. If you believe information about a child has been submitted inappropriately, contact our office. If you provide information about another person, make sure you are authorized to do so.
